To support chronic disease management, it is essential to ensure that patients are totally involved in their own care through continual self-assessment and monitoring. The AxSys solution provides the platform and tools to extend care into the home.
We offer innovative solutions for remote assessment and monitoring of patients, through either a secure web based Patient Portal or via a telephone based questionnaire delivery system that is simple, intuitive and fully interactive, requiring only a touch tone telephone. Patients who require monitoring for any reason, including drug compliance and chronic disease management, can now have 24-hour contact with their care provider.
Our remote monitoring solutions are fully integrated with the multi-disciplinary care record. This means that they can be set up to interrogate and write back into patients' case notes so that questionnaires can be delivered dynamically for each patient, based on data in their individual notes.
